The ALPHA Center serves all individuals regardless of ability to pay. If you are concerned about your ability to pay for services, please call our office to find out how you can qualify for the Sliding Scale Fee Program or other assistance programs.

For your first visit you need to bring:

Photo ID
Social Security Card
Official printout of current medications
Any referral information
Insurance or Medicaid card
